The benefits listed are subject to applicable eligibility rules and waiting periods and may be dependent on the employee's status and/or hours worked.
Medical Plan -Clear One
WCHCD pays 100% of the employee premium for all three medical plans, including vision coverage. $300 deductible Preferred Provider Plan that provides comprehensive coverage including alternative, vision and prescriptions. If you enroll your dependents in the medical plan, WCHCD will contribute $125 per month to your dependent premiums. Higher deductible insurance plans are available.
Vision Insurance - Clear One
Pays 100% of exam at preferred providers. Insurance covers up to $100 for frames, $56 for single vision lenses, $84 for bifocals lenses, etc.
Prescription Drug Plan -Clear Choice
A three-tier prescription drug plan that pays $5/40/60 for prescription drugs from a preferred provider. Mail order is available for a three month's supply at one co-pay amount for each prescription.
Employee Assistance Program (EAP)- Clear ONe
This program provides members with access to confidential services to help resolve problems that may interfere with life. You are offered up to 6 visits per issue, per year, at NO cost to you. This program covers dependents and all household members, related or not.
Wellness Program - Clear One
This program is designed to help you adopt health habits and covers all family members 18 years or older who are enrolled in a CCHP. Members access these benefits by visiting www.clearchoicehp.com and clicking the "Wellness" link.
Dental Insurance -Advantage Dental
Dental Plan emphasizes preventive care with three different incentive levels.
Section 125 Flexible Spending Account - Manley Services
Child care/insurance premiums/health care expenses paid with pre-tax dollars.
Air Link Membership
Air Link (Air/Ground Combination) membership for employee household. Cost is $90.00 per year. Paid by the District for all full-time and part-time employees.
Life Insurance - Regence Life and Health
Life Insurance Benefits: In the event of death, the life insurance paid through this plan is
$10,000. Life insurance coverage is for employee only, paid by the District.
Employee Paid Life/Accidental Death and Dismemberment (AD&D)
You may elect from $10,000 to 300,000 of additional life insurance for you or your spouse and up to $10,000 for your dependent children. Each employee and spouse will be subject to evidence of insurability.
Short-Term Disability - Regence Life and Health
In the event of an off the job injury or illness that prevents you from working for a period of time, our STD plan provides income benefits as long as you are physician certified as disabled. The benefits are 70% of your income to a maximum of $500.00 per week for Class I employees and $1,000.00 per week for Class II employees (Professional Staff) to a maximum of 26 weeks. Paid by the District for full time employees. Part time employee's premium can be pro-rated if they choose to participate.
403 (b) Retirement Program
The District has a voluntary retirement plan with several investment options available to all employees classified full or part time. Beginning in year two of service the District will match the employee's contributions of 1% to 9% depending on length of service.
